Title: Using your story for research
Who is involved
● Human Line Project (Montreal, Canada) collects and stewards stories.
● Accredited university research partners work with Human Line to study AI-related spirals in the public interest.

Why we are asking
Your story can help researchers understand who is affected, how spirals unfold and what might prevent harm. This pilot uses the story you already shared. We will not contact you directly unless you opt in below.
What will be used
● The information you provided to Human Line (basic demographics and your free-text narrative).
● No email addresses or names are shared with researchers. Human Line keeps contact details securely and separately.
How your information will be handled
● Human Line will remove names and other direct identifiers and redact obvious indirect identifiers before any sharing with researchers.
● Researchers will work in a secure, access-controlled environment.
● Results will be reported only at group level.
● No identifiable case stories will be published in this pilot without your explicit permission.
Risks and benefits
● There is no payment and no direct clinical benefit, but your contribution may help improve our understanding of this emerging mental health risk and and guide the development of early recognition, intervention and future prevention.
Your choices
● Taking part is voluntary. You can withdraw by emailing [contact@humanline.org] any time before the data are combined for analysis or publication. Already published aggregate results cannot be changed.
If you shared about someone else
● You can consent to the use of your account of what happened. Please avoid names or details that could identify the other person. This consent does not give permission to contact them.

2. Research use of my de-identified story
I consent to Human Line sharing my de-identified story and answers with accredited university research partners listed at [humanlineproject.org/research-partners] for academic research on AI-related harms. I understand the analysis will be at group level and that no identifiable case details will be published in this pilot.
3. Optional: future contact via Human Line
[ ] Yes, Human Line may contact me about opportunities to get involved in future research. Researchers will not receive my email unless I later choose to share it. Ticking this box does not commit me to involvement in any future research but allows me to be contacted by Human Line if suitable research opportunities arise.
4. Optional: de-identified verbatim extracts from my story or transcripts
[ ] I consent to the use of de-identified verbatim extracts from my story or any chat transcripts I later provide in academic papers, reports, presentations and public-facing materials about AI-related spirals. I understand quotations increase the risk of recognition; names and obvious identifiers will be removed; wording may be
lightly paraphrased or details omitted to protect privacy. I can withdraw this permission any time before publication by emailing Human Line.
